Filing a Death Benefit Claim
To file a claim, complete the Death Benefit Claim Form provided by Prudential. Attach a certified copy of the death certificate, proof of beneficiary designation, and any other requested supporting documents. Submit the form through the insurer's online portal, by mail, or through a licensed agent.
Processing a Policy Surrender or Cash Value Withdrawal
For a surrender, fill out the Surrender Request Form and provide proof of identity. If you are withdrawing part of the cash value, submit the Withdrawal Request Form along with a statement of the desired amount. Prudential will calculate the surrender charge, if any, and the net payout.
